The Crest Clinic
The Crest Clinic provides medical and pain management services. Our services include face to face and video medical consultations, prescriptions, referrals, investigations and procedures.
We do not provide any emergency or urgent treatment. Therefore, please do not book an appointment if you think your health problem is urgent or an emergency. Do not book an appointment under any circumstances if you are experiencing a medical emergency, such as chest pain, or severe abdominal pains (this list is not exhaustive, and is for general guidance only).
If you think there is any chance that you or the person you are assisting has an urgent or emergency health need you should immediately dial 999 or seek alternative emergency medical services.
About Us
The Crest Clinic is owned by Medinow Ltd, a Limited company registered in England and Wales with the company number 12568903 and whose registered address is Kemp House, 152 To 160 City Road, London, England, EC1V 2NX.
We are regulated by the Care Quality Commission (“CQC”) which is the independent regulator of all health and social care in England. The CQC monitors, inspects, regulates and reports on all medical clinics, and other health & social care services.

Terms and Conditions
Please read these Terms carefully and make sure you understand them. By using The Crest Clinic, you confirm that you are accepting these Terms and Conditions in full. You will be asked to agree to these before using our services. If you do not understand, accept and agree to these Terms and Conditions we are unable to provide you with a consultation or any of our services.
The Crest Clinic may change these terms at any time by posting changes online. Please review these terms regularly to ensure you are aware of any changes made. Your continued use of The Crest Clinic after changes are posted means you agree to be legally bound by these terms as updated and/ or amended.
Consultants Fees
The consultation fees are £249 for a virtual or face to face appointment with a Pain Specialist.
Prescriptions
You understand there is no guarantee that you will be provided a prescription for any medication and acknowledge the decision to issue a prescription is at the sole discretion of your doctor in the current consultation.
Our doctors may issue you with a prescription if appropriate. The prescription(s) provided are private prescriptions only and are not NHS prescriptions. You accept that private prescriptions are fulfilled at a charge, unique to the pharmacy. The prescription(s) you receive are only valid for use at legal U.K. pharmacies. You agree and understand that any prescriptions you receive are solely for your personal use. We will discuss the pharmacy partner to be used for delivery or fulfilment of the medications. Please note, The Crest Clinic is not responsible for ensuring that you receive the medication you are prescribed.
Appointment Requirements
Appointments for Pain Specialist services are for patients who are at least eighteen (18) years of age and have the legal right, capacity and ability, to agree to these Terms and Conditions on your own behalf.
You agree to book your appointment at The Crest Clinic providing accurate information, including but not limited to your name, postal address, mobile telephone number and email address.
The Crest Clinic reserves the right to refuse an appointment to any individual and recommend alternative services including NHS GPs or Specialists, whether or not the user has previously successfully registered, visited or used any of the medical services.
To allow us to provide the Services to you in a way that protects the interests and safety of all our patients, we will need to carry out checks to verify the identity of any person wishing to use telephone and video consultations, to make sure you are who you say you are. We will ask you to show a photo ID at the consultation to verify your identity (passport or driving licence).
If you fail to provide us with this ID information, we may be unable to provide any services to you and, we may cancel any consultation that you may have booked by that point in time.
You will be responsible for providing us with the address and contact details for the NHS practice that you are registered with to enable us to share your records.
Our doctors will provide you with a letter for your NHS GP outlining the contents of your consultation as discussed at your appointment and include any recommendations for you.
You will then forward this letter to your NHS GP and they will assess the information and refer to their NHS commissioned services as they deem appropriate. The Crest Clinic cannot hold any responsibility for whether your NHS GP or their commissioned services will accept the recommendations of The Crest Clinic doctors.
Pre-payment policy
We require pre-payment/deposit to secure the booking of appointments with all of our clinicians. Appointments are subject to the cancellation policy (see below).
We cannot refund any medication, vaccinations, or treatments once they have been administered or dispensed.
The costs for additional tests, service(s) or procedures must be paid in full to the doctor, nurse or health care assistant at the end of your consultation and prior to the test/service being undertaken. Patients requiring a hospital procedure must pay for the procedure prior to admission. Procedures will be refused if a patient has not paid in full.
Rescheduling
You may reschedule your appointment free of charge up to 48 hours before the time of your appointment. You can do this via the link in your email booking confirmation or by calling or emailing us.
If your booking is less than 48 hours before your scheduled appointment you may reschedule but you will lose any pre-payment for your initial appointment.
Cancellation policy
If you are unable to attend your appointment you agree to let us know at the earliest opportunity. Appointments can be cancelled up to 48 hours before your scheduled appointment time. We will charge an administration fee of £25 and our team will contact you on one occasion to help reschedule your appointment. You will not be eligible for a refund if you fail to attend on the rescheduled appointment
If you cancel your appointment or reschedule less than 48 hours before the scheduled appointment time you will not be eligible for any refund on any pre-payment.
Bookings made less than 48 hours before the scheduled appointment time which are cancelled will forfeit any pre-payment.
Late Policy
To provide the best possible service to all our patients, we ask you to contact us if you are running late so we can make all necessary arrangements to ensure you are seen by our medical team. As our clinics are very busy, you may be asked to reschedule your appointment or wait for an available slot so that all our patients receive the best possible service.
If you are more than 10 minutes late for your appointment without our prior agreement, we may consider you to have failed to attend, in which case, you will not be eligible for a refund of your pre-payment.
Failure to attend
If you fail to attend your appointment or are more than 10 minutes late to your booked appointment or cancel your appointment within 48 hours of the scheduled appointment, you will be deemed to have missed your appointment. You will not be eligible for a refund of any pre-payment.
Refunds
Refunds for eligible pre-payments will only be made to the card originally used to make the original booking. We will process your refund within 7 days of the request. Once we have processed your refund, please allow up to 10 working days for the refund to appear on your statement as the time taken by your bank to credit the refund on your statement is outside of our control.
We cannot refund any medications, vaccinations, procedures, tests or treatments once they have been administered or dispensed.
Administration fee
Appointments and home tests that qualify for a refund are subject to an administration cost of £25 which will be deducted from the amount refunded. This is to cover our reasonable costs.
Privacy
By accepting these Terms and Conditions, you acknowledge that you understand, have read and will abide by The Crest Clinic’s Privacy Policy (all as updated from time to time in accordance with its terms), which are part of these Terms and Conditions by reference.
The Crest Clinic may contact you by email to the email address provided in your account, by written communication to the postal address provided in your account or by a general notice on the system.
Applicable law
These Terms and Conditions are governed by English law and you can bring legal proceedings in respect of our Services in the English courts.
Intellectual Property
The names, images and logos identifying The Crest Clinic or third parties and their products and services are subject to copyright, design rights and trademarks of The Crest Clinic and/ or third parties.
Referrals for laboratory and diagnostic services
Any laboratory tests (e.g. bloods and urine tests) needed will be advised and you will be informed of the price. If appropriate, our doctors will refer you to a private diagnostic service to have imaging tests (such as X-ray, Ultrasound scans, MRI, CT scans). This is usually done as a letter to the provider requesting the diagnostic tests agreed and outlining the medical reasons for your request.
It will be your responsibility to take this letter to your local private provider and arrange the investigation. You will be entirely responsible for any payments required by the provider for the test. These costs are not covered by the consultation fee with the doctor.
Results of the investigations will generally return directly both to yourself and to doctor who has referred you. A follow-up appointment will be arranged to discuss your results.
Please note that our doctors cannot accept any responsibility for the accuracy of the results nor the experience of obtaining these investigations as this is the responsibility of the diagnostics service provider.
